Maybe I should restructure my question: Is it possible for someone to provide me with the format for a treatment? I do not need a sample, I just want to make sure that I put the treatment together correctly.

If this is for a feature film then you'll want to break the treatment down into 3 acts. Within each act you'll describe the events that occur in each scene. Separate scenes by paragraph. For example:
"My Great Feature"
Treatment
by Joe Schmoe
ACT I
A paragraph for scene 1.
A paragraph for scene 2
...
ACT II
A paragraph for a scene.
A paragraph for a scene.
...
ACT III
A paragraph for a scene.
A paragraph for a scene.
...
That's it. The more detail you put into each scene, the easier it will be for you to write your script when it's time.
