If a reality show is being shot in a public bar or nightclub does the music playing in the club require licensing from the artist?

If it is recognizable. If you are able to decipher any part of the song that could make it recognizable, even if only to a professional audio recordist, then it needs licensing. So in your case almost certainly yes.
