I wonder why the survey is so limited in terms of age and gender. Why not simply include two more questions regarding age and gender so you'd get more data?
My own son fits into the demographic listed, but the fact is that I see MORE movies overall than he does and if he does see something, it's likely that I'm either going with him or he's getting recommendations from me about movies he may not have heard of before from friends or current marketing.

Your last question has a serious problem. You have limited the number of choices for respondents, and didn't allow for "other":
8. Which of the following are typical features of the action films you primarily watch?
Shooting
Car chases/ vehicle stunts
Explosions
Martial Arts/ Wire-Fu (Rush Hour, The Matrix, etc)
Brawling/ Fist-fighting
You would have likely been surprised by the number of people in that age group who watch action features for the twists and surprises of the plot line, and not any of the physical action on screen. With the choice of answers such as yours, you'll never find out, and your survey loses some validity.
The rest of the survey looks fine, though.
