Script format/presentation

Hi I know the format of a script (font, size etc) but don't really know the physical presentation of it is supposed to be like. The things I mean are:
Paper
Binding
What should on the front cover

Can anyone help or direct me to a site

Standard letter size paper - I believe it's called A2.

Three hole punched, two brass brads

The title of the script and name of the writer are centered on the page.
In the lower right the contact info.

I'm in the states. I don't know the exact type of paper. I could be wrong. Maybe it's A4.

What is the standard paper size called in the UK?

Yes there is A2 but it is about 42cm x 59cm. A4 is the standard and that is what I thought it would be printed on, I pressume that is the same size as standard letter size. Does anybody know?

Also I always remember scripts I saw in movies being printed on yellowish paper. It is meant to be white though isnt it
