Note: I consider this forum a great place to share ideas, to get help and to help others. A digital home away from home. I have asked many questions and got many answers and would like to thank everyone. I'd also like to share a release form I came across today. There are many variations, but add this one to your folder..mix and match this with existing forms you may already have.

p.s. A word of advice. If the actor/actress does not have a lawyer present and you make the release form to "formal" or full of complicated jargon they MAY possibly be able to get out of it. There is such thing as...I believe it is called "plain english" contracts. Meaning they can bring it to court that they were screwed because they were not aware of certain clauses. Make sure they understand it. Maybe at the bottom you might want to add something that states: By signing this binding agreement/binding contract I am fully aware of its contents and etc. etc.

I authorize _________________ (the "Producer"), the Producers agents, successors, assigns, and designees to record my name, likeness, image voice, sound effects, interview, and performance on film, tape or otherwise (the "Recording"), edit such Recording as the Producer may desire, and incorporate such Recording into the Film and all related materials, including but not limited to literary, promotion, and advertising materials. It is understood and agreed that the Producer shall retain final editorial, artistic, and technical control of the Film and Film content. The Producer may use and authorize others to use the Film, any portions thereof, and the Recording in all markets, manner and media, whether now known or here after developed, throughout the universe in perpetuity. The Producer and/or Producer's successors and assigns, shall own all rights, title and interest, including the copyright, to be used and disposed of, without limitation, as the Producer shall determine.

For the sake of argument what is the purpose of the SS#? With ID theft the talent might not want to give the ss#.

Just a thought

I think the same form would be used for a paid gig and in that case the SS# would be for tax purposes. For a non-pay gig they can probably just leave it blank.

that's true, never thought about that. I think it is just one more way to verify they actually signed it and it's not fake. Then again if they became a celebrity and it was worth it, you could have an expert test the the signiture by analyzing the pressure applied, comparing it to other stuff they signed etc.

I'm not going to use that because all my gigs as of right now are TFP.

The SS number should never be on the release. All actors used in a
movie need to sign a model (or image) release form like this one
- which is pretty standard - if you pay them or not.

If you are paying then there are different forms that need to be
signed - and those will include SS#. But never, on a model
release.
