I need help with 2 forms.

I am presuming the TV Cue Sheet is used for the music in the film? My composer owns copy-right, but I own the usage. I just don't understand the 'Tape accession number' & 'Part' etc.

As for the Copyright Return form, is this just for footage? I own the rights to the footage. It asks for details of owner for stuff created not specifically for the film (but everything was made for it), so do I leave this blank? Do I have to list every single shot?
